Consular Section - Immigrant Visa Unit Manila, Philippines MEDICAL EXAMINATION Instructions for Immigrant, "K" and "V" visa applicants ALL VISA APPLICANTS, REGARDLESS OF AGE, NEED TO COMPLETE A MEDICAL EXAM AT THE ST. LUKES MEDICAL CENTER EXTENSION CLINIC (SLMCEC) BEFORE THE VISA INTERVIEW. THE EMBASSY WILL NOT PROCESS YOUR APPLICATION WITHOUT THE RESULTS OF YOUR MEDICAL EXAMINATION. FOR YOUR MEDICAL EXAMINATION, BRING YOUR PASSPORT, APPOINTMENT LETTER (FOR IMMIGRANT VISA APPLICANTS) OR APPROVAL NOTICE OF THE PETITION (FOR "K" AND "V" APPLICANTS), A 2" X 2" PHOTO AS WELL AS ALL OTHER DOCUMENTS THAT THE EMBASSY HAS REQUIRED YOU TO SUBMIT AND. AN SLMCEC PERSONNEL WILL CHECK AND ASSEMBLE THE DOCUMENTS FOR YOU. I. FIRST COME, FIRST SERVED BASIS Go to ST. LUKE'S MEDICAL CENTER EXTENSION CLINIC, which is located at 1177 J. Bocobo
St., Ermita, Manila. Telephone: 521-00-20 or 521-8647 and 524-6083. Office
hours: 7:30 AM to 11:30 AM 85 1:00 PM to 4:30 PM (Monday through Friday except Philippine holidays). Applicants are examined on a first come, first served basis. Female applicants may request for a female doctor to perform the examination. The results of the examination will be given to you to submit during your visa interview. Due to space limitation at the clinic, we advise you not to bring companions. II. MEDICAL EXAMINATION FEE The cost of the medical exam is US$95 for adults (15 years and older) and US$67 for children under 15 years of age. Repeat X-rays will cost US$7each. These fees must be paid in peso equivalent at the prevailing U.S. Embassy exchange rate. III. X-RAYS Children younger than 15 years old will not need a chest X-ray or serological test unless required by the doctor. According to the U.S. Public Health Service (USPHS), chest X-rays for pregnant women are not dangerous if taken with the proper shielding. If you have any previous X-rays, please bring them to along for comparative study. IV. HUMAN IMMUNODEFICIENCY VIRUS (HIV) TEST A blood test check for HIV is required if you are at least 15 years old. The results of your test will be provided to a consular officer. If you test positive for HIV you will have to report this to the health authorities of the Philippines. Note: HIV is the virus that causes acquired immune deficiency syndrome or AIDS and impairs a person's immunity against disease. AIDS refers to illnesses that persons infected with HIV may contract and is usually fatal. |

of the United States of America Consular Section - Immigrant Visa Unit Manila, Philippines Important vaccination notice for "K" and "V" visa applicants : The United States Immigration and Nationality Act (INA) requires all foreign nationals
lawfully admitted for permanent residence to be vaccinated against certain
preventable diseases. - - Persons who are issued "K" and "V" visas will therefore be asked to comply with this INA requirement before they are allowed to adjust status in the United States from a nonimmigrant to a Lawful Permanent Resident (LPR). While K and V visa applicants technically are not subject to the INA vaccination requirement, they are encouraged to meet the vaccination requirements before admission to the United States. During your medical examination at the St. Luke's Medical Center Extension Clinic, the panel physician will help you determine if you have met the vaccination requirements for: o Mumps o Measles o Rubella o Polio o Tetanus and diphtheria toxoids o Pertussis o Influenza Type B (HIB) o Hepatitis B o Varicella o Pneumococcal o Influenza Consult your regular health care provider about your vaccination history and ask
for a copy of your vaccination record, if one is available. If you do not have
such a record, the SLMCEC panel physician may help identify which vaccinations
you still need. However, only civil surgeons of the Bureau of Citizenship and Immigration
Services (BCIS) can determine which of the listed vaccinations are medically
appropriate for you. Upon arrival in the U.S. you need to contact the
local BCIS office for the names and addresses of designated civil surgeons who
can assist you in completing the vaccinations requirements. |
